A clear path from a first referral through to the next steps, with the home, the room, the move-in date and the rate agreed in writing before anyone commits to anything.
What the person gets
Housing, and help keeping it
Kindred runs the homes. Keeping a home is more than having a room in it, so helping residents hold onto their housing is part of the program.
In practice that is the ordinary, practical side of staying housed: working out who to call, getting to the right resource, the paperwork. It is general help with living somewhere, offered by the people running the house — not a service delivered to a plan, and not something a resident is assessed for.
Where that stops
Kindred is not a care provider and does not represent residents. We will not speak or act on someone’s behalf with your organization, a landlord, a court, or anybody else, and we do not take over anything you are doing for them. If you are the person coordinating their care, you stay the person coordinating their care.
Please do not send us case files
We are a housing operator, not a clinical or care provider. We have nowhere appropriate to hold an assessment, a treatment plan, a medication list or a diagnosis, and holding one would create a record about the person you are placing that serves nobody — least of all them.
If a document arrives that we should not have, we will tell you and ask you to resend without it.
What we do need
- Who you are and which organization you are with
- Broad timing and housing logistics
- Which planned community interests the person
- The best way to arrange a future conversation, with their agreement
What we do not
- Diagnoses, medication or treatment history
- Clinical assessments or care plans
- Case notes or progress reports
- Anything you would not be comfortable putting in an email
